PERSONNEL RECORDS



The Shelby County School System shall maintain a file in the Human Resource Department for each employee. The file shall include the following sections:

  1. BUSINESS SECTION
    The business section shall include all applicable licenses and/or certificates, standard scores, academic records, academic transcripts, and application forms.

  2. EVALUATION SECTION
    The evaluation section shall include all written evaluations.

  3. ADDITIONAL EVALUATION MATERIAL
    Complaints, reprimands, suggestions for improvements, observation reports by supervisors or principals, and commendations may be placed in the employee's file under the following conditions:

    1. The comment is signed by the author.

    2. The employee is notified by the Human Resource Department that such comment is available in the Human Resource Department prior to placement in the employee's file.

    3. The employee shall have an opportunity to offer a written response and have it placed with the comment.

  4. MEDICAL FILE
    All medical information is maintained in a separate file.
