Kindergarten Registration

Bring the following requirements:

•Certified birth certificate (child must be 5 years old on or before September 30, 2008)

•Social Security Card

•Tennessee Child Health Record signed by a physician indicating proof of the following (a Temporary Tennessee Certificate of Immunization may be submitted, if the physical examination has been completed and immunizations have been started)

o4 doses of DPT-DtaP-DT-DTP/Hib-Td

o4 doses of Polio (OPV/IPV)

o2 doses of MMR one given after 1st birthday

o3 doses of Hepatitis B

oChicken Pox vaccine or proof of having had the disease

oProof of a medical exam within the past 6 months

•Two of the following as proof of residency

oReal Estate Tax receipt

oMortgage information or Rental Contract

oCurrent MLGW bill or turn on receipt

oMilitary Letter of Assignment

Grades 1-5 Bring Two Proofs of Residency (listed above)
