Please provide originals of two of the following five items showing the parent/guardian's name and address to prove residence this 2005-06 school year (originals ONLY - copies will not be accepted):

• Sale Contract (closing papers within 30 days or title)
• Real Estate Tax Receipt
• Mortgage Information (excluding a contingency agreement)
• An MLG&W Bill or Turn-On Receipt (most current bill - July or August)
• A Rental Contract

These are the ONLY proofs thata can be accepted.

**Parents/Guardians who are living with a relative/friends OR who live in a house that does not belong to you but are not renting will be required to provide the Notarized Letter for Proof of Residence (this form is available in the office) which must be signed by the person residing at the address. In addition, both the homeowner and the parent/guardian living with them will be required to appear in person at the school with the two items of proof for the homeowner. The parent/guardian will be given 10 days to provide additional proof of their own that they reside at that residence. Home visits may be made at the discretion of the appropriate administrator.

Federal Court Orders prohibit the enrollment of any child in Shelby County Schools unless they are living with a parent/guardian who resides in the area served by the Shelby County School System.

Grades or other school records may not be released until assessment is paid in full. (Policy 6103)
