Registration Information for 2007-2008

All parents will have proof of residence approved before continuing with registration.  This is for new and returning students.

Registration for Grades K-5 will be held on Tuesday, August 7th,  2007 from

7:30AM to 12AM  and  4PM to 7PM   Transportation will not be provided for students.

A) Each student must provide the school the original (copies not acceptable) of two

of the following documents:

1) Sales contract (contingency agreements not accepted)

2) Real Estate Tax Receipt

3) Mortgage Information

4) Current MLG&W bill or Turn-On Receipt or a municipality water bill

5) A Rental Contract or Lease

6) Military Housing Letter (for military people living on the base, this letter is all that

is required.

Kindergarten

--must be 5 years old on or before September 30, 2007

--birth certificate

--proof of medical examination

--proof of immunization

B)Required immunizations for students in Kindergarten

    Four doses of DPT, Four doses of Polio, Two doses of MMR, 3 doses of

    Hepatitis B.  Proof  of chickenpox immunity, either the vaccination or proof that

    the has had chicken pox.

    Required immunizations for students in 1, 2, 3, 4, 5

    Four doses of DPT, Four doses of polio, Two doses of MMR

C) Social Security Number
