Headshot Request Form

From October through June 2022, the studio will be open twice a month for employee headshots between the times of 9 am and 12 pm.

Please select a date and time for your portrait. The photography staff will confirm your appointment when they receive your request. Note: Individual headshots are only allowed once a year.

Photo studio is located on the 2nd floor in the Teaching and Learning Academy (TLA) studio side. Please pull around to the back parking lot and come through the double doors. Someone will come down to escort you upstairs.

Time slots are from 9 am-12 pm every 10, minutes For example: 9 am, 9:10 am, 9:20 etc
