Registration, Fall 2008


New Student Registration:
To expedite new student registration, please print and complete the forms below.  Submit this information at the time of registration.

All students new to Collierville High School who did not register at Collierville Middle School or Schilling Farms Middle School in the spring and did not register during the summer should report to the school Library at the time scheduled for their grade. New students should bring (1) proof of residence, (2) the most recent report card or transcript, and (3) their immunization record.

Returning Students:
Grades 10-12
Students again will need to bring two proofs of residency .

Grade 9
Students again will need to bring two proofs of residency .
(See information on Proofs of Residency below)

Students will report directly to assigned classrooms with their proofs of residency; parents will report to the auditorium for an orientation meeting.

Once schedules are issued and verification of residency is completed, the freshmen will attend all classes on an abbreviated schedule of five minutes
per class with five minutes between classes.

All teachers of freshmen will be in their classrooms to introduce themselves and to hand out any information (supply list, class rules, etc.) students
should know for the first day of class. This will conclude around 7:45 PM.
Parents will be in the auditorium for a “parent orientation” meeting until 7:45 PM. The 9th grade counselor and all administrators will provide information to parents to assist in the transition from middle school to the high school setting.

ATTENTION ALL PARENTS AND STUDENTS -
PROOF OF RESIDENCE

Federal Court orders and Shelby County School Board Policy require that students who live with their parent/guardian in the Shelby County School District may attend Shelby County Schools.

Therefore, on Registration Day all returning students will be required to provide proof of residence during homeroom. Photocopies will NOT be accepted.

All students must provide two (2) Proofs of Residence from the list below:

  1. Sales contract (closing paper or title)
  2. Real estate tax receipt
  3. Mortgage information (contingency not acceptable)
  4. MLG&W bill or turn on receipt
  5. Rental contract or Military Letter of Assignment

    OR

If living with someone else, must provide:

  1. A notarized letter for proof of residence with two (2) proofs of the homeowners' residence from the list above
  2. The homeowner and parent/guardian are required to appear in person at registration.
  3. Parent/Guardian will have 10 days to provide 2 items of proof showing their name and the correct address.

If the student does not live with both parents, a copy of guardianship papers is required.
