Historical Records and Transcripts

Principals, as a reminder, prior to any adjustments to student historical records (Transcripts), approval must be granted by the district office. Historical Change of Data requests must be completed and submitted in Ivanti, formerly FootPrints, along with supporting documentation (i.e., copy of student’s transcript with affected course highlighted, copy of report card if necessary and the Historical Change of Data form with all required signatures) per the Middle/High School Grading protocol, page 10. Curriculum & Instruction (scsk12.org) 

All requests shall be accompanied by written Justification. Such justification shall include the student’s participation in any class, school, district program, interventions, or make up assignments.

Requests that do not meet the above criteria will not be approved. They will be returned for corrections and must be resubmitted. Changes to historical records without following the applicable protocol and approval process, will be reported to the appropriate supervisor.
