MATERNITY/ADOPTION LEAVE


Maternity Leave

  1. The employee requesting maternity leave must file a written request with the Human Resource Department on a form provided by the Human Resource Department at least thirty (30) days prior to the requested date of leave. The dates provided for this leave must be from a date certain to a date certain. A physician's statement, on a form provided by the Human Resource Department, must accompany the request for maternity leave.

  2. The maternity leave may be extended to a later specified date as long as the total period of leave, including the requested extension, does not exceed one (1) year. A written request for extension of leave must be filed with the Human Resource Department at least thirty (30) days prior to the expiration of the original request.

  3. Prior to returning from maternity leave, the employee shall submit to the Human Resource Department a doctor's statement confirming her physical fitness to return to duty.


Adoption Leave

  1. The employee requesting adoption leave must file a written request with the Human Resource Department on a form provided by the Human Resource Department as soon as the employee is notified of the date to receive the child. The dates provided for this leave must be from a date certain to a date certain.

  2. The adoption leave may be extended to a later specified date as long as the total period of leave, including the requested extension, does not exceed one (1) year. A written request for extension of leave must be filed with the Human Resource Department at least thirty (30) days prior to the expiration of the original request.
