The following items must be provided during the application process: NO EXCEPTIONS

• Certified Birth Certificate - Not Mother’s Copy
  

• Physical & Shot Record on TN School Immunization Certificate (Yellow or Green Card)
  

• Social Security Card
  

• Proof of income (2)
  

(1) Income tax form 1040 or 1040A
(2) W2 form
(3) Verification of pay by employer (current pay stubs)
(4) AFDCF Public Assistance payments


• Proof of residency for your school zone (2 of 5). If parent/guardian lives with someone else you must complete the Notarized Letter for Proof of Residence (Click Here).
  * Must include current MLG&W bill or turn on receipt and one of the following
  

(1) Real estate sales contract
(2) Real estate receipt
(3) Mortgage information
(4) Lease/Rental contract

Please Note: All of the above items are required in order for the application to be complete. Each Pre-K child must be brought to the application process with their parent or guardian.
