Registration will take place on Tuesday, August 4, 2009 from 7:30 a.m. to 12:00 p.m. and 4:00 p.m. to 7:00 p.m. In order to eliminate as much confusion as possible, the registration process will be as follows:

ALL STUDENTS

Proof of residence is required of all students at registration and may be required of any student during the school year. At registration, everyone MUST provide two of the following five items:

Sales contract (contingency agreements are not accepted)

Real Estate Tax Receipt

Mortgage Information

Current Memphis Light, Gas & Water bill or turn-on receipt

Rental contract

The parents/guardians and students who are living with someone else must meet these requirements:

1. Provide a notarized form letter for proof of residence, which can be picked up at the school office.

2. The owner/lessee must accompany the parent/guardian to school for registration on August 4th with two items of proof from the five accepted items listed above.

The parents/guardian must bring to registration 2 of the following:

1. Drivers License (with current address)

2. 3 pieces of mail with forwarding stickers

3. Payroll check ( current address)

4. Car Registration (current address)

5. Voter Registration (current address)

If these items are not received on registration day, the child will not be allowed to register.

RETURNING STUDENTS

Students who attended Crosswind during the 2008-2009 school year will follow the procedures detailed below:

1. Proof of residence will be verified in the following locations:

Last name beginning with A-I in the Art room.

Last name beginning with J-Q in the Library.

Last name beginning with R-Z in the Computer Lab.

2. After residence has been verified, you will be given a Registration Approval form which is to be taken to the Gym. There you will receive a packet to complete and return on Monday, August 10 , 2009.

STUDENTS NEW TO CROSSWIND AND KINDERGARTEN STUDENTS

New students and kindergarten students should proceed directly to the Cafeteria where proof of residence will be verified and registration completed.

Students who were previously enrolled in another school system in Tennessee may enroll pending our receipt of their academic and health records. Students transferring into the system from out of state or from a nonpublic school must provide a permanent Tennessee Certificate of Immunization or an up to date Temporary Certificate.

Kindergarten students must be 5 years old on or before September 30, 2009, and the following items MUST be provided:

1. Certified copy of the Birth Certificate (not mother's copy)

2. Proof of recent medical examination (within the last 6 months)

3. Permanent Tennessee Certificate of Immunization (green form) or the Temporary Certificate of Immunization (Kindergarten through 12th grade must have two doses of MMR, and all Kindergarten students and 7th graders must have proof of Hepatitis B immunization).

4. Social Security Number.

SCHOOL SUPPLY PACKETS AND PLANNERS

School supply packets and student planners will be sold in the Gym during registration. Supply packets include all supplies needed per grade level. The cost of the packet is $50.00 and includes the student planner. Planners may be purchased separately for $10.00. Cash only for school supplies.

TEACHER ASSIGNMENTS

The teacher/student classroom assignments will be made after the completion of registration. The assignments will be posted at the front and back doors of Crosswind on Friday, August 7, 2009 at 5:00 p.m. Parents and students may check the listing at that time or during the weekend for the assignments. Please be sure your child knows his/her teacher and room number prior to the first day of school, Monday, August 10th.

It is imperative that you register your child on Registration Day, August 4, 2009. Correct numbers could mean the difference in whether we add or lose teachers. If you plan to be out of town on registration, please let us know before you leave. Assignments are based on projected enrollment and are subject to change within the first 20 days.
