Registration Requirements for ALL students - new and returning students

 

Federal Court orders and Shelby County School Board Policy require that only students who live with their parent/guardian in the Shelby County School District may attend Shelby County Schools

All Students

Everyone will be required to provide the following documents for proof of residency on registration day:

a current utility bill and a mortgage statement.

If you live with someone else, you must bring a notarized form with the person you reside with to registration. The homeowner will need to prove residency and the parent will also need to provide proof of residency. Notary forms will be available in the front office on August 1.

If a student does not live with both parents, a copy of guardianship papers is required.

Requirements for Enrollment in Kindergarten

1. Certified birth certificate

2. A Tennessee Child Health Record* signed by a physician indicating proof of the following:
*Equivalent forms for immunization records will not accepted.

4 doses of DPT -DTaP-DT-DTP/Hib-Td,
one of which was given on or after the fourth birthday. If age seven or older three doses are required.

4 doses of Polio (OPV/IPV)
If the 3rd dose was given on or after the fourth birthday, the 4th dose is not required.
However, if a combination of IPV/OPV (2IPV/2OPV) is used all 4 doses are needed regardless of age.

2 doses of MMR with 1 dose being given after the first birthday.

3 doses of Hepatitis B.

• 1 dose of the chicken pox or the dates of the disease

• Proof of a medical examination within the past 6 months (Policy 6101).

A child entering kindergarten must be five years of age on or before September 30, 2007
